Market engagement for a support and care service to the Jewish Community at London Borough of Redbridge. The aims and objectives of the main elements within the Service are: • To provide an early intervention and prevention service to people with disabilities, people with Dementia and their carers living in Redbridge who are not eligible for other Authority funded services under the Fair Access to Care Services (“FACS”) criteria; • To provide an assessment service for Clients referred to the Service, the type of assessments carried out shall include: - Specialist assessments across all client groups; and - Holistic assessment of need and planning over short, medium or long-term provision.
