Tender description

East Sussex County Council (‘the Council’) Adult Social Care and Health Department (ASCH) is contracting for a single provider to provider a ‘Carers Centre’ for East Sussex from October 2019 for 5,5 years, up to March 2025, with a further option to extend the contract for up to 24 months (to March 2027). Working with the 3 Clinical Commissioning Groups (CCGs) ESCC want to promote, maintain and enhance people’s well-being and independence in their communities so they are healthier, more resilient and less likely to need formal health and social care services. Commissioners are seeking strategic partners to engage in co-design of outcomes and service models: we want to collaborate with delivery partners to achieve long-term benefits and innovation based on mutually desired outcomes.
