Tender description
The main elements within the service are: — provide early intervention and prevention service to people with disabilities, people with Dementia and their carers within the Jewish Community who are not eligible for other Authority funded services under the Fair Access to Care Services (‘FACS’) criteria, — provide an assessment service for clients referred to the Service, the type of assessments carried out shall include: —— specialist assessments across all client groups, —— holistic assessment of need and planning over short, medium or long-term provision. Further information regarding the service delivery and requirements can be obtained via www.londontenders.org The service is to provide general and specific support to older people, people with disabilities, people with Dementia and their carers within the Jewish community in the Authority. The main elements of the service are: — a carer specific provision, — an open provision. Carer specific provision: The carer specific provision shall account for 50 % of the total service under this provision. The focus for the carer specific provision shall be on carers of people with disabilities and people with dementia within the Jewish community. All services delivered under this provision shall be led by the carers’ own assessment of needs as carried out by the provider. The provision shall meet the Service outcomes and the carer’s personal outcomes. Support shall be relevant to every area of the carer’s life and caring responsibility, and shall aim to assist carers maintain their caring roles and live quality lives of their own. Open provision: The open provision service shall account for 50 % of the total Service under this provision and shall be open to both carers and persons cared for. The service shall be provided after a joint assessment of need of both carers and persons cared for. The aim of the open provision is to enable flexible and joint planning of support and care for carers and persons cared for so that their needs can be assessed and met holistically, taking their interdependencies into consideration and ensuring a seamless service catering for the needs of the carer as well as the requirements of the cared for at the same time.
