Greater Huddersfield CCG: The Nook Group Practice, Primary Care Medical Services

Tender description

Following receipt of notice from the previous contractor, NHS Greater Huddersfield Clinical Commissioning Group appointed an interim provider to provide safe, high quality; effective GP led primary medical care until 31st March 2020. The CCG is now looking to appoint a provider under an APMS contract for a period of five years. Opening hours and service delivery from 8:00 to 18:30 Monday to Friday, (except statutory bank and public holidays). Interested Providers will be required to provide core primary care services which will include GP and clinical sessions in line with patient demand The population currently served are in the following geographic areas: • Golcar • Salendine Nook The average number of patients registered with the practice is expected to be between 2220 and 3440.
