Flexible care and support to 5 adults with autism spectrum disorder/learning disabilities and associated complexities moving from in-house residential provision into the community in a core and cluster living environment. The contract will provide flexible care and support to 5 adults with autism spectrum disorder/learning disabilities and associated complexities moving from in-house residential provision into the community in a core and cluster living environment. Ongoing flexible care and support will be required 24 hours a day, 7 days a week, to assist the service users to achieve their assessed personalised outcomes and maximise their potential. The service users will be accommodated in newbuild flats in the Dedridge area of Livingston. Although the contract focuses on the ‘Core’ component of the service in the first instance, the proposed model forms part of a re-design of learning disability services in the West Lothian locality and the ‘cluster’ element will be developed at a later date by WLHSCP whereby care and support will be offered to residents in geographical proximity to the Core.
