Tender description
Hull City Council will be awarding 9 grants to fulfil the following outcomes from the Prospectus, and via the application process we will identify the best organisations to deliver services to meet these specific outcomes. Organisations are free to apply for more than one grant. Services commissioned in this Prospectus will include: • Support, Advice and Prevention following a Stroke, • Support and Advice for People with Dementia and/or Alzheimer’s and their Carers, • Preventative based Day Opportunities, Falls Support Services and Respite and Outreach Services for Older People, • Pre-Employment Training and Support for Adults, • Drop In services and Day Opportunities for people with mental health issues, and • Carers Services All services commissioned in this Prospectus are designed to support and enhance information, advice, peer based advocacy, prevention and support based services. Services developed will enable individuals and families to be more self-reliant, to have more choice, independence and control over their lives and to seek help earlier and in doing so reduce or delay their need for statutory Social Care and Health based services. The process for 2019 will be as follows:: Prospectus 1: 19th June 2019; grant opportunities published; 12th July deadline for submission of applications; August 1st 2019 award letters for grants. All services commissioned in this Prospectus are designed to support and enhance information, advice, peer based advocacy, prevention and support based services. Services developed will enable individuals and families to be more self-reliant, to have more choice, independence and control over their lives and to seek help earlier and in doing so reduce or delay their need for statutory Social Care and Health based services. The Prospectus offers opportunities for existing and new organisations to work in partnership with Hull City Council and Hull CCG to improve outcomes for local people. An example of an outcome, or the things that will be different for people as a result of the commissioning award, might be that older people are supported to live independently and safely in the community.
