Tender description

Dorset County Council (Dorset Council wef 1.4.2019) and NHS Dorset clinical commissioning group procured the Framework in 2017 and it commenced on 1.12.2017. In accordance with its terms, this is the first general re-opening to new entrants. The Framework covers 3 segments of service provision: — care and support at home, (Segment 1), — community innovation (Segment 2), and — care and support in a care home (Segment 3). Segment 1: Lot 1: care and support at home, including nurse led complex care; Lot 2: day opportunities; Lot 3: intermediate care; Lot 4: rapid response; Lot 5: roaming nights; Lot 6: short breaks; Lot 7: live-in care; Lot 8: extra care; Lot 9: children's care and support at home and community support. Segment 2: Lot 1: creative solutions utilising community assets. Segment 3: Lot 1: care and support in a care home; Lot 2: day opportunities; Lot 3: intermediate care. Lot 1: care and support at home (including nurse led complex care). Lot 2 Day Opportunities Lot 3 Intermediate Care Lot 4 Rapid Response. Lot 5 Roaming Nights Lot 6 - Short Breaks Service Lot 7 - Live-in Care Lot 8 - Extra Care Lot 9 - Children's Care and Support at Home and Community Support. Segment 2 Lot 1 - Creative Solutions Utilising Community Assets. Segment 3 Lot 1 - Care and Support in a Care Home (with or without Nursing). Segment 3 Lot 2 - Day Opportunities. Segment 3 Lot 3 - Intermediate Care.
