Market engagement for the exercise on referral and weight management service at London Borough of Redbridge. The provider shall ensure patients shall be referred to the service by their GP, and other health care professionals. The provider shall ensure that once the patient receives a referral from their GP/other agreed healthcare professions, a qualified exercise specialist shall design a tailored exercise programme, for the patient to complete over twelve (12) sessions. The provider shall ensure patients experience the benefits of a physically active lifestyle and shall be encouraged to continue physical activity to maintain this change over time. The service Further information regarding the service delivery and requirements can be obtained via www.londontenders.org The provider shall ensure the service covers the whole of Redbridge, based at three (3) locations which are easily accessible by public transport and appropriately temperature controlled. The Provider shall note the three (3) locations are: Wanstead Leisure Centre, Fullwell Cross Leisure Centre and South Woodford. The main requirements (but not exclusively) of the service are as follows: — provide twelve (12) session intervention for individuals with a range of medical conditions in leisure centres throughout the Borough of Redbridge at a nominal cost to the participant, — provide leisure centre-based exercise options to those referred onto the service, — to carry out pre- and post-programme assessments on all appropriately referred patients, — provide a range of exit routes from the service including subsidised leisure centre membership or community-based classes or sessions, — to provide follow-up advice, support and other activities to encourage participants to remain active, — to maintain liaison with general practitioners, local authority and other partners to ensure the service is operating effectively, in line with local needs and the national quality assurance framework for exercise referral systems, — to maintain a database of participants and carry out regular monitoring and post programme evaluation, — to provide feedback to GP’s about the patient they referred to the service, and — to promote the service, encourage uptake, and regularly publicise the service in the borough.
