Tender description
NHS Sheffield Clinical Commissioning Group (CCG) and Sheffield City Council (SCC) have been working in partnership with Sheffield Teaching Hospitals NHSFT (STH) to: — help reduce emergency admissions to hospital, — support timely hospital discharge, and — wherever possible, discharge people to their own home or normal place of residence. Somewhere else to assess is a model that supports individuals who are medically fit to be discharged from hospital, but would benefit from a period of assessment in a care home setting while their long-term care and support needs are identified.
