Trafford Council and Trafford CCG have been awarded Greater Manchester Transformation Funding for a range of health and social care projects. As a result of this funding, Trafford Council has committed to a programme of work to transform the way in which homecare and reablement at home services are commissioned and delivered. The Transforming Homecare Programme will focus on improving the quality of homecare provision, enhancing the role of the homecare workforce and ensuring that homecare providers are adequately resourced to achieve the above. The future of care at home is both a national and a local issue, and is complex in its nature. It requires a cultural change and impacts on the wider health and social care system. Who should attend: • Voluntary sector organisations • Organisations that offer and/or co-ordinate training relevant to the homecare workforce; those working with the elderly; those with dementia etc. • Organisations that work with hospitals; enable patient discharge; have a transport service; offer practical support to make people safer in the home Keywords: reablement, homecare, vulnerable
