Lambeth Clinical Commissioning Group (CCG) will be procuring a community diabetes service consisting of the following elements: • Referral management • Advice and guidance for patients and healthcare professionals • Community Clinics for type 2 diabetes care • Self management support and structured education provision for patients at high risk of developing diabetes, newly diagnosed or with pre-existing diabetes • Virtual clinics in primary care • Education and learning for primary and community care We propose to procure a community diabetes service for 2 years to deliver an integrated community-based diabetes service via a multi-disciplinary team which is evidence based and cost effective and supports patients with type 2 diabetes to achieve better health outcomes We are looking for potential providers to attend a market engagement event to discuss and feedback on the proposed service delivery model and the outline service specification.
