Tender description

The Manchester support service will deliver a single holistic non-medical, person centred low level, cross tenure housing and customer support service which will promote independence, choice and wellbeing. The service will facilitate hospital discharge and integrate with health and care services to help prevent hospital admissions and readmissions. The service to be delivered will comprise the following aspects: — provision of a Handyperson’s Service including a Dementia Aids and Adaptations programme and child accident prevention programme, — home repairs support service (including grants and low cost loans) and also including a boiler repair/replace service, — provision of an all-encompassing non-clinical hospital discharge support service; administration of a home support fund. Manchester City Council and NHS Manchester clinical commissioning group bring together the commissioning of health, adult social care and public health into a single partnership arrangement. Our aim is that the benefits of a single commissioning voice will include: — an integrated and proactive health and social care, — large scale transformation of services, — a focus on prevention and sustainability, — better use of our collective assets. The service to be delivered will comprise the following aspects: — provision of a Handyperson’s service including a dementia aids and adaptations programme and child accident prevention programme, — home repairs support service (including grants and low cost loans) and also including a boiler repair/replace service, — provision of an all-encompassing non-clinical hospital discharge support service; administration of a home support fund. The service will be integrated with the neighbourhoods and localities of Manchester and therefore locally managed making use of local facilities. The service will ensure the Council meets statutory duties, such as the Equality Act (2010), the Accessible Information Standard (2016) and a commitment to work according to the new General Data Protection Regulation (2018). The service will also have a prevention and wellbeing focus in line with the principles in the Care Act (2014).
