Accommodation based support for up to 720 units for persons aged 60 and above to help maintain tenancies and to provide a flexible and person centred service delivering different levels of service based on individual needs. This will be based upon a low, medium and high package of support which can flex to meet individual needs in times of crisis such as short term health impacts, providing orientation for new tenants or those returning from hospital. Develop and facilitate the skills required by service users for independent living and by enabling access to other services. Provide preventative measures to enable individuals to remain in their home. Facilitate reduction in isolation and loneliness. Ensuring that service users feel safe and secure within their accommodation. Maintenance of wellbeing. In regards to support/isolation, etc. Each tenant will be assessed (including any risks) and a support plan formulated.
