The services required will provide innovative and evidence based practice that supports people to develop their independent living skills and work towards fulfilling personal goals, aspirations and well-being. The services will also encourage individuals to form a greater connection to their communities, reducing isolation and social exclusion. A further benefit, in line with our programme to integrate health and social care locally, will be preventing hospital admission and enabling quicker discharge from hospitals. Provision of these services should therefore delay or prevent people escalating to higher levels of need. This enables the Local Authority to discharge its duty under the Care Act 2014 to provide preventative services and increase the well-being of residents. This will also deliver on the Mayoral commitment to promoting independence in adult social care and delivering high quality services to those who need support. See medium level specification.
