Tender description
The delivery of an older people’s hub and a disability hub within the borough. Contracts will be awarded to one or two lead providers. There will be a lead provider for each hub. Each of the hubs will be required to have a network of providers working with them. The hub model will assist the Contracting Authority to manage its “front door’ by supporting individuals living within the borough to access information, advice, navigation and facilitation support that will link them with community opportunities and services across Southwark to support their independence and wellbeing. The Contracting Authority is seeking to appoint an experienced, competent lead provider for this hub who can manage a network of providers and also work in partnership with the Contracting Authority to deliver an information, advice and sign posting service to Southwark residents. The key deliverables are: — tailored support and navigation to client centred services, — information and advice that meets an immediate need, — preventative offers that continue to be relevant and responsive to people's changing needs, — strong community links that facilitate joint working across organisations, — group sessions and activities, and — opportunities for community engagement, development of skills and support to residents. The Contracting Authority's overall aim for the hub model is to ensure that people who may have complex and multiple problems are able to access the support they need through early intervention to stop issues from escalating.
