Tender description

The service is to provide person-centred and outcome focussed housing related support to young people aged 16 to 25 including looked after children, care leavers and young people under the care of the youth offending team who have an established local connection to the Isle of Wight. The provision will deliver accommodation and support on the Isle of Wight which aims to increase independence skills and promote engagement with education and employment through flexible support packages. The service provided should offer a range of accommodation models in various settings providing accommodation based, personalised and outcome focused support. Moving from foster care/residential in planned way as they prepare for independence; separated children seeking asylum, typically placed as part of the national transfer scheme. Care leavers. Young people who were in care for 13 weeks after their 14th birthday and are in care on their 16th birthday. Accommodation and support for young people with complex needs (16-25 years of age). Accommodation for parents and children with the parent aged between 16-25 years whilst providing housing related support to maintain independence and help to secure accommodation in order to move on to independent living. Young people aged 18-25 who have become homeless and require housing related support to secure accommodation, in order to move on to independent accommodation. Housing Related Support for Young Parent(s) (16-25 years of age). Young people who require housing related support in the community to either find suitable accommodation or to maintain their tenancy. No accommodation is required under this lot.
