Age Well Services are a key part of Wandsworth Council’s approach to transforming adult social care and managing demand by enabling people to retain, gain or regain independence. 1.1) These Age Well Services are a key part of Wandsworth Council’s approach to transforming adult social care and managing demand by enabling people to retain, gain or regain independence; targeting preventative resources most effectively and ensuring value for money; 1.2) At the heart of this approach is a vibrant voluntary and community sector (VCS) working in partnership with the Council, the NHS and other stakeholders (including businesses and faith groups) to help residents access local services that enable them to improve or maintain their overall well-being and stay independent for longer; 1.3) We are seeking a provider, or providers, who will bring innovative thinking to achieve the following outcomes for older residents: — reducing social isolation, — increasing confidence in managing major life changes, — increasing contribution to their community, — improving physical activity and healthy eating, — equipping them with skills required for later life including digital skills, — supporting residents who are unpaid carers, — skill sharing and volunteering, — co-producing intergenerational activities/interventions and whole family approaches to help deliver above outcomes, — physical self-care and health awareness related to ageing; 1.4) The model is not centre based and will require the successful provider(s) to focus on using existing assets to support older residents in a range of locations in the community including children’s centres and other community venues.
