Tender description
Services include: The practice clinical pharmacist scheme: this service aims to improve the quality and safety of prescribing; deliver financial targets as set by the CCG; engage GP practice teams with the CCG medicines management team and develop the role of the clinical pharmacist/pharmacy technician in primary care. Frailty: the objective of the pathway is to formalise the identification, support and interventions for the frail elderly who often end up in acute crises and place demand on the health and care system. GP out of hospital schemes — a traditional Local Enhanced Services (LES) where clinicians offer a local appointment to avoid a hospital attendance. The services described are being made available to all primary care providers in the CCG's patch.
