Tender description

Provision of a community based end of life care assessment and coordination service for patients within Sutton CCG area. This is a nurse led service supported by a multidisciplinary team of specialist palliative care community nursing and a social worker. The service will support the reduction of unnecessary hospital admissions for individuals who are in the last 12 months of their life. Direct Award of contract with Sutton Health and Care Provider Alliance (SHCPA) through Epsom and St Helier University Hospitals NHS Trust to include the delivery of end of life care assessment and coordination on 2 years’ basis with an option to extend these arrangements for up to a further 2 years extension to allow for alignment with Sutton Health and Care integrated care contractual solution currently anticipated from 2022. Recognising its obligations and responsibilities under the NHS (Procurement Patient Choice and Competition)(No 2) Regulations 2013, the CCG has assessed the most appropriate route to awarding this contract, having regard to the necessity to secure continuity of these essential patient services and has concluded that the provider will assure the safest solution and being considered the only technically capable provider at this time. The integrated solution will be delivered through the Alliance partners: Epsom and St Helier University Hospitals NHS Trust; Sutton GP Services Ltd; London Borough of Sutton; and South West London and St George's Mental Health NHS Trust.
