Tender description

The CCG approved the commissioning of step up/step down beds by utilising extra supported housing and care schemes. These are short stay beds available for 6 weeks providing care closer to home for communities across HRW. They support individuals to maximise their potential and remain as independent as possible whilst avoiding long stays in hospital. Staff within the extra care scheme work alongside the integrated health and social care team to ensure patients’ care and support needs are met and this contract ensures that this is in place for the step up/step down bed in Thirsk.
