Good and Services. The Service Provider by this agreement will provide to the Council and NHS Wiltshire the Service, to supply a range of equipment and practical services in the home enabling independent living to Customers within the administrative County of Wiltshire for the Council and to patients registered with a Wiltshire GP who reside no more than 10 miles outside the Wiltshire boarder for NHS Wiltshire. The Service Provider by this agreement will provide to the Council and NHS Wiltshire the Service, to supply a range of equipment and practical services in the home enabling independent living to Customers within the administrative County of Wiltshire for the Council and to patients registered with a Wiltshire GP who reside no more than 10 miles outside the Wiltshire boarder for NHS Wiltshire. To provide assessments and advice with qualified Occupational Therapists (OT) and Occupational Therapist Assistants (OTA) and has the facilities for people to appraise a range of different equipment and daily living aids. Visitors can refer themselves for an appointment and health and social care related organisations can signpost to the service.
