Market Engagement Event for Supported Care Services for NHS North and South Norfolk CCGs.

Tender description

North and South Norfolk Supported Care Services aim to keep patients at home and help them to maintain independence for as long as possible. The service is aimed at frail patients, such as those who have unstable long term conditions or at risk of acute infections, requiring an immediate health and care response to avoid being admitted to hospital or support when discharged home to avoid readmission. The service provides immediate response, short term wrap around reablement care with nursing and therapy oversight. NHS Arden and Greater East Midlands Commissioning Support Unit (AGCSU) on behalf of the commissioners, NHS North Norfolk CCG and NHS South Norfolk CCG, would like to inform the market of a proposed upcoming procurement process, for the provision of supported care services delivered under a new contract from 6.1.2020 and to invite providers to attend a Market Engagement Event to be held at The Willow Centre, Cringleford, Norwich on 15.4.2019 from 14:30 to 16:30.
