NHS West Hampshire Clinical Commissioning Group (the CCG) is undertaking the implementation of this care homes for nursing care, residential care and hospital placements framework (with windows) and subsequent call-off on behalf of the following Authorities: West Hampshire CCG, South East Hampshire CCG, Fareham and Gosport CCG, North Hampshire CCG and North East Hampshire and Farnham CCG.
