Tender description

Norfolk County Council wishes to add a new lot to an existing framework, the framework for Supported Living. Lot 2 is for providers of support to people with a learning disability and or autism who display behaviour that challenges, including those with a mental health condition. Providers under lot 2 will provide supported living care and support packages for individuals leaving hospital or at risk of admission to secure in-patient facilities. This application is for Lot 2 — Transforming Care Project and is for providers of support to people with a learning disability and/or autism who display behaviour that challenges, including those with a mental health condition. Transforming care is all about improving health and care services so that more people can live in the community, with the right support, and close to home.
