Tender description

North Tyneside Council is seeking to commission a suitably qualified and experienced Provider(s) to deliver a Social Prescribing Service which enables individuals with mild to moderate mental health conditions and those who are socially isolated, to live full and active lives by connecting them with appropriate opportunities and activities in their community. The Service will enable health and care professionals to refer adults for personalised goal setting and support into non-clinical community services and networks; and signposting to additional sources of information, advice and support as appropriate.
