NHS HMR CCG and Rochdale Borough Council are proposing a new integrated service model for children’s community health services that will support our drive to improve outcomes for children and young people. The service will operate as a single entity, with each individual service element contributing to and supporting delivery of a shared set of outcomes. The service model at this stage will include the following existing services: — Children’s Acute and Ongoing Needs (CAONS), — 0-5 universal public health services (health visiting), — 5-19 universal public health services (school nursing). The services are to be delivered as part of a comprehensive 0-19 years (0-25 years to meet the CCG’s statutory obligations regarding specialist educational needs and disability (SEND)) seamless service for children and young people. The deadline for responses to this PIN/market testing document is 16:00 on 15.4.2019.
