Tender description

East Sussex County Council (\"ESCC\") intends to commission a cross county programme to improve wellbeing and reduce health inequalities across the life-course through community centred and asset based approaches. A single provider is being sought to provide the Asset Based Wellbeing Programme (\"the Service\") commencing 1 October 2019 and ending 31 March 2025, with a further option to extend the contract for up to 24 months (to 31 March 2027). The Service will be provided in; Eastbourne Borough; Hastings Borough; Lewes District; Rother District; Wealden District. The Service will be targeted in communities where individuals at most risk of poorer health and wellbeing outcomes are more likely to live and where Service is most likely to reduce health inequalities. Delivery of this service is through a Strategic Partner model. The Strategic Partner (\"the Partner\") will hold the contract with East Sussex County Council and will be responsible for the delivery of the Primary Service. The Primary Service can be provided either solely by the Strategic Partner, or through network of providers of the primary services (\"the Network\"), whereby there is a contractual arrangement between the Partner and any member organisations of the Network. Both the Partner and Network will be expected to participate in a Commissioning Platform (\"the Platform\"), together with ESCC as equal partners, sharing decision making and commissioning services to deliver against community identified outcomes (\"Secondary Services\") using asset based commissioning practices and principles. Neither the Partner nor Network will be permitted to be recipients of grants to deliver Secondary Services.
