Tender description

The purpose of the service is to: — support people who do not have the skills to live independently, — prevent homelessness, repeat homelessness and offending, — improve service users’ health and wellbeing. 1.1) Our services are: — commissioned to work holistically with a range of vulnerable people with specific needs, most having multiple needs, — focussed mainly on secondary prevention (where primary prevention is preventing the occurrence of problems, secondary is preventing problem escalation, and tertiary prevention is reducing the severity of problems), — for some services we commission, focussed on tertiary prevention (i.e. Rough Sleepers Service, and support for people with Complex and Multiple Needs). This service is mainly focused on secondary prevention, but for some of the service users, the focus will be on primary or tertiary prevention. 1.2) The purpose of the service is to: — support people who do not have the skills to live independently, — prevent homelessness, repeat homelessness and offending, — improve service users’ health and wellbeing, — reduce harm to service users and others and thereby avoid unnecessary use of emergency and crisis services, — ensure that individuals will move on from the service, settle into the community and manage a tenancy successfully.
