The CSSH Framework is for the supply of care and support to people who fall into the service categories below. Providers can submit a bid to provide services in one or more Service Categories: — service category 1 Older People Personal Care and Support — service category 2 Mental Health — service category 3 Learning Disability — service Category 4 Learning Disability Highly Complex — service Category 5 Autism — Service Category 6 Physical Disability — service Category 7 Dementia — service Category 8 Nursing (Level 3 nursing tasks) Care and Support for Older People aged 65 and over who can no longer live at home due to increased care needs which may include physical or mental illness, disability or physical frailty, bereavement, an emergency or the needs of informal carers. The CSSH Framework will incorporate a simple and cost effective process of calling off services from providers, either by mini-competition or direct award. The Council are carrying out call offs for care and support services in a number of extra care housing schemes in conjunction with the framework application process. Providers are asked to submit innovate proposals through the mini-competition process for 5 Extra care housing lots that support people aged 55 and over within existing accommodation. The 5 lots fall into Service Category 1 and include: — Lot 1a: Rosewood Court, Wellingborough, Northamptonshire, NN8 1LQ, — Lot 1b: Sunley Court, Kettering, Northamptonshire, NN15 7RJ, — Lot 2a: Imperial Court, Rushden, Northamptonshire, NN10 9AF, — Lot 2b: St Crispin’s Court, St Crispin Dr, Northampton NN5 4RB, — Lot 3: Langdale Court, Wellingborough, Northamptonshire, NN8 3XA. Providers that are interested in providing services to the extra care housing schemes must submit the appropriate lot questionnaire at the same time as they apply to the CSSH Framework. Those providers that only want to apply to the CSSH framework must just submit the framework questionnaire to become a framework provider. It is anticipated that the CSSH framework will support people to move from a residential care setting, or their own homes, into suitable accommodation where care and support can be effectively accessed. The CSSH framework will also help to achieve the quality, continuity, affordability, availability and comprehensiveness for the services required. Suitable providers are therefore encouraged to apply for future opportunities and propose new models for implementation within the county so that independent living for vulnerable people can be promoted. The CSSH Framework will be ‘open’, which means that once the initial providers have been awarded a CSSH framework contract the Council can open up the framework to other providers in the future, should more providers be required. This will be at the Council’s discretion. Adults with significant and enduring Mental Health needs including diagnoses of schizophrenia, bi polar disorder, personality disorder and enduring anxiety and depressive disorders who require support to reach an optimum level of independence and whose needs may fluctuate depending on their level of recovery. Individuals may have forensic histories, substance misuse and/or chaotic lifestyles and have identified risk histories that will need monitoring and regular liaison with other professionals. The CSSH Framework will incorporate a simple and cost effective process of calling off services from providers, either by mini-competition or direct award. Those providers that only want to apply to the CSSH Framework (and not submit a Tender for the 5 extra care schemes immediately available) must just submit the framework questionnaire to become a framework provider. Call offs offered in this area will be for adults between the ages of 18 and 64 with a long term physical impairment or illness including: — people born with a physical disability, for example people with spina bifida, muscular dystrophy, spastic quadriplegia or sensory impairment, — people who suddenly acquire a trauma based disability, for example spinal injury or acquired brain injury, — people who acquire a disability from a long term condition, for example multiple sclerosis or rheumatoid arthritis, — and additional complex needs arising from multiple conditions including mental health issues or challenging behaviour. This an umbrella term used to describe a range of progressive neurological disorders. Dementia can affect adults of any age usually affects people aged 55 and over becoming more common as age increases. There are many different types of dementia, of which Alzheimer’s disease is the most common. Some people may have a combination of types of dementia including Alzheimer’s disease, Vascular dementia, Frontotemporal and Korsakovs. The CSSH framework will incorporate a simple and cost effective process of calling off services from providers, either by mini-competition or direct award. All generic social care tasks and skills plus: — cough assist, — laryngectomy care, — tracheostomy care, — oral suctioning or suctioning via tracheostomy, — non-invasive ventilation (Bi-pap/CPAP), — administration of oxygen, — catheter care (including supra pubic), — gastrostomy feeding (including PEG care), — application of TED stockings or compression hosiery, — seizure management (administration of buccal midazolam/rectal diazepam), — stoma care, — bowel care, — end of life care, — medication administration (including medication not in a dossett box/monitored dosage system e.g. short course antibiotics, prescribed ointments creams lotions, nebulisers, inhalers, liquid medication), — feeding individuals with compromised swallow.
