PRJ601 - Waltham Forest Multi-Lot Community Direct GP Access Services AQP

Lot 1: Community Direct GP Access Ambulatory ECG, BP and Plain ECG Service Contract. The Commissioners are seeking suitably qualified providers via an Any Qualified Provider (AQP) process to deliver a Community Direct GP Access Ambulatory ECG, BP and Plain ECG non VAT-able Service. The service will be community based, and providers must deliver services in the London Borough Waltham Forest. An evening and weekend level of service must also be provided, along with provision for home visits to patients that are housebound. The service will: ? Receive referrals from Waltham Forest GP practices ? Triage referrals for appropriateness ? Provide the above diagnostic services ? Provide reports to the referring clinician ? Onward refer to secondary care services where appropriate The Contract value has been estimated at £646k* over the entire lifetime of the contract term (5 year period, i.e. three years, plus two year extension), and across all providers. The contract will run for 3 years, plus an additional 2 years if the extension option is exercised by the commissioners. If the commissioners exercise the extension option, a window for new providers to be part of the qualified providers list will also be offered. *Based on M7 17/18 FOT Lot 2: Community Direct GP Access Echocardiogram Service Contract. Waltham Forest CCG currently provides Community Direct GP Access echocardiogram service via an AQP contract. As required by procurement regulations, the CCG is offering the opportunity for any qualified providers to apply to join the AQP agreement to provide echocardiogram services. The service will be community based, and providers must deliver services in the London Borough Waltham Forest. The service will: ? Receive referrals from Waltham Forest GP practices ? Triage referrals for appropriateness ? Provide the above diagnostic services ? Provide reports to the referring clinician The Contract value has been estimated at £4.2m* over the entire lifetime of the contract term (5 year period, i.e. three years, plus two year extension), and across all providers. The contract will run for 3 years, plus an additional 2 years if the extension option is exercised by the commissioners. If the commissioners exercise the extension option, a window for new providers to be part of the qualified providers list will also be offered. *Based on 16/17 OT Lot 3: Community Direct GP Access non-obstetric ultrasound (NOUS) Service Contract. Waltham Forest CCG currently provides Community Direct GP Access NOUS service via an AQP contract. As required by procurement regulations, the CCG is offering the opportunity for any qualified providers to apply to join the AQP agreement to provide NOUS services. The service will be community based, and providers must deliver services in the London Borough Waltham Forest. The service will: ? Receive referrals from Waltham Forest GP practices ? Triage referrals for appropriateness ? Provide the above diagnostic services ? Provide reports to the referring clinician The Contract value has been estimated at £780,000 over the entire lifetime of the contract term (5 year period, i.e. three years, plus two year extension), and across all providers. The contract will run for 3 years, plus an additional 2 years if the extension option is exercised by the commissioners. If the commissioners exercise the extension option, a window for new providers to be part of the qualified providers list will also be offered. *Based on 16/17 OT Qualified bidders can submit bids for one, two or all three of the contracts.
