Tender description

NHS North Lincolnshire CCG GP practices provide a number of services to patients within the primary care setting. The Authority are intending to award contracts for a 1 year period only from 1 .4.2018 to ensure continuation of health services can continue within a primary care setting to North Lincolnshire's patients, whilst a larger service review is conducted regarding the delivery of services from 1.4.2019. GP practices receive GBP 3 per registered patient, calculated based on 1 April list size. Payments to be made as follows: — 50% to be paid in Q1m — 25 % to be paid 1 September; — 25 % to be paid 1 January. Elderly Care Fund. — Work with relevant associated health and social care professionals to deliver a multi-disciplinary care package that meets the needs of the patient, — Ensure that the physical and psychological needs of the patient are recognised and responded to by the relevant clinician in the practice, — Ensure the patient aged 75 years and over has access to a health check, — Develop MDT Care packages/plans for patients aged 75 and over where clinically appropriate.
