Tender description

The Tender relates to the provision of a Family Support including Transition Supports for Children with a Disability Service. The HSCB plan to appoint one organisation to provide the Service. The objectives of this procurement are to provide: 1) Short break services to children with a Disability including those transitioning into adult services (up to age 21); 2) Personal Development programmes with mentoring support to children with a Disability who are in the process of transitioning from children’s services (with particular emphasis for those aged 16 to 21 years); 3) Disability Awareness training to those organisations to which a young person is transitioning in respect of their particular need; 4) A support and link with the Children and Young People’s Strategic Partnership (CYPSP); Children with Disability Regional Group to inform the strategic planning of this group; 5) A model of partnership working with statutory and non-statutory bodies.
