Managing Deconditioned Patient Pathways. There is growing evidence highlighting the importance of exercise in chronic disease. The benefits to health of a physically active lifestyle are well established and a sedentary lifestyle plays a significant role in worsening disease morbidity, increasing frailty and in decreasing function and independent living. Despite a recognised need for public health interventions such as Exercise Referral Schemes, encouraging sedentary people with a medical condition to become more active is often limited by the availability of effective and timely activity-based interventions close to the patients’ home. This proposition is to implement an integrated national activity-based program from referral to treatment, with a dataset to compare the different interventions for the management of de-conditioned patients with chronic disease, in conjunction with the Respiratory Health Implementation Group (RHIG) and NHS Wales.
