Tender description

The service will be expected to provide the following types of support: — Face to face contact with service users. — Give advice, information and signposting. — Provide an out of hours emergency contact number. — Primary prevention. — Secondary prevention. — Person centred to achieve the desired outcomes. The service will work with service users and their carers/families towards achieving greater independence in the community and to minimise barriers to social inclusion in a sensitive manner. Local commissioners reserve the right to challenge the provider if they judge any individual intervention is no longer delivering preventative outcomes i.e. the service user has become dependent on the service. The service will ensure a person centred approach to delivery to encourage service users to be creative and help them take positive risks instead of being risk averse.
