The aim is to ensure that the overarching aim of health visiting / specialist community public health nursing services for children under 5 is to protect and improve the health and wellbeing. Of children and their families. The school nursing service covers maintained schools, academies and free schools; it includes child health surveillance, health promotion, health protection, health improvement and support outlined in the healthy child programme 5-19 which includes: — the role of school nursing in transition for school-aged children, for example transition between health visiting and school nursing, and into adult services, — the role of school nursing and the contribution to safeguarding, — the role of the school nurse in supporting vulnerable children and those not in school, e.g. children in care, young carers, home educated children or young offenders, — the support offered as part of the troubled families programme. The Hounslow health visiting service will deliver the Health Visiting elements of the HCP delivery of the 5 health reviews: — The health visiting service will offer comprehensive health and development reviews to families, — each health review will use validated, evidence-based assessment tools including Ages and Stages Questionnaires (ASQ) at 27 months. Ensuring a consistent understanding of family strengths, needs and risks; providing parents with the opportunity to discuss their concerns and aspirations, — assess child growth and development, communication and language, social and emotional development and — detect abnormalities. In LBH, the health visiting service will have a strong emphasis on the antenatal contact and family health needs assessment, providing a strong foundation for future intervention and support. Provide an integrated Public Health Nursing Service with a named school nurse for every education setting; with strong links to key partners who can easily contact the service. — work with local authority commissioners to ensure that clear care pathways exist between health visiting and school nursing teams, — work with school leaders and school improvement services to identify population health needs, — ensure and be able to evidence that the experience and involvement of families, carers, children and young people will be taken into account to inform service delivery and improvement 10, — champion and advocate culturally sensitive and non-discriminatory services that promote social inclusion, dignity and respect, — demonstrate the impact of the service provided through improved outcomes and service user feedback, — work with and support the NWL Health Protection Agency and National Health Protection Agency, as and when required, on health protection matters.
