Tender description
The overall aim of the service is to provide one to one PBS case work for high priority individuals across ONEL, and to provide a programme to train providers and professionals across ONEL in the principles and application of PBS. The objectives of the service are as follows: • The needs and preferences of people with a learning disability and/or autism should be the focus of all we do. Support delivered as part of this programme is expected to be person-centred, allowing people to have choice and control and empowering them and their circle of support. • PBS should be embedded and used as a preventative approach, ensuring that individuals are supported in the optimum way at the earliest opportunity in order to prevent future placement breakdown, hospital admission, and unnecessary strain on the person and their family. • A ‘whole life’ approach should be taken, embedding skills that provide an evidenced quality of life for the person and allows them to develop in ways meaningful to them. • Case work will be delivered in partnership with an individual and their circle of support. Where this involves families or carers, the provider shall recognise the expertise and knowledge of their family member. All work with families and carers shall be supportive and encouraging. • The provider shall be expected to work closely with local community teams, recognising their expertise and the knowledge they have in regards to individual cases. The work provided should complement and enhance existing services offered by the community teams. • Significant provider liaison will be required in order to achieve engagement and changes in ways of working. The provider shall seek to increase the skills and capacity of providers identified as part of the project in order to better support people with learning disabilities and/or autism in the community.
