Expression of Interest for individual package

Tender description

This is an expression of interest request. We are looking for a package of care and support for a young man currently residing in residential care within County Durham. Whilst the preferred location of any service would be within County Durham we would be willing to consider a service in and around the Sunderland area. Either residential care or a supported living model would be considered although we would require a spacious environment. He is currently in a 28 day notice period due to the transition from child to adult provision. Therefore any prospective provider must be able to respond quickly and have the available staffing resources in place. He has a diagnosis of autism, severe learning disability and refractory epilepsy. He also displays very challenging behaviour which can vary from hitting, spitting, kicking out, grabbing, throwing items and smashing items. His behaviour can be aimed at himself, his peers, others or his environment. He is a very complex young man who requires strict routines and familiar staff. He currently requires 2:1 support at all time. He has good family relationships. He takes medication for his epilepsy and has a VNS implant. An epilepsy protocol is in place and he experiences seizures on a daily basis which can sometimes require rescue medication.
