Tender description
Croydon Council is seeking competent providers to deliver the Invitations aspect of the NHS Health Checks Programme. The NHS Health Check programme is a national systematic vascular risk assessment and management programme established to assess an individual’s risk of heart disease, stroke, and diabetes and kidney disease. The target group are those aged between 40 and 74 who have not previously been diagnosed with a vascular condition. All eligible persons are offered the opportunity to have a Health Check every five years. Further details are available in the specification.
