Tender description

Leicestershire County Council wishes to seek suitable providers to deliver a pilot scheme for 6 short term residential care beds for individuals who require a short period of assessment to enable them to return home or to be moved into the correct setting for their needs. Typically, this scheme will be required following a hospital stay or where the individual has experienced a health or social care crisis in their own home. The focus of the unit is on social care interventions and approaches, rather than health care ones; the assessment will focus on an individual’s social and environmental situation, helping to resolve social problems and achieve a maximum level of choice and control. A dedicated case manager will be responsible for the case during the admission. This procurement will be using the Light Touch Regime tender. This Service is for adults who reside in the County of Leicestershire; it is for individuals who are assessed as medically stable and consequently able to transfer from a hospital setting or community environment. Due to the pilot status of the service, it is likely that the service model, service standards and reporting requirements will change over time (funding may also change in a directly proportional manner in response). LCC and the provider will adopt a partnership working style to improve both outcomes and shared learning. A dedicated case manager will be responsible for the case during the admission.
