Tender description
The Authority intends to commission a Carers Assessment and Support Service in Lancashire, a Lancashire Advocacy Hub and a Lancashire Carers Advocacy Service as separate contracts within one procurement process. The tender can be found on the Authority's Oracle Sourcing Portal's under the RFQ Number: 15223634. Amended versions of the tender are denoted at the end of the RFQ Number by a comma followed by an amendment number. Search results will display any and all amended versions of the tender. The latest version is determined as being the RFQ Number with the highest amendment number. You may only make a submission against the latest amendment. Where you have made a submission against a tender that is subsequently amended you will be required to resubmit prior to the submission deadline. Carers Assessment and Support Service (North Lancashire). The Service Provider will support and enable the health and wellbeing of Carers by ensuring Carers are identified at an early stage; are supported to identify themselves as 'Carers'; have access to a range of personalised information, advice and services that they need to support them in their role; are offered a Carers' Assessment and Support Plan; have an annual Review of their Carers' Assessment and Support Plan; are supported to take regular breaks from their caring role; are supported to improve their own health and wellbeing. Carers Assessment and Support Service (Central Lancashire). The Service Provider will support and enable the health and wellbeing of Carers by ensuring Carers are identified at an early stage; are supported to identify themselves as 'Carers'; have access to a range of personalised information, advice and services that they need to support them in their role; are offered a Carers' Assessment and Support Plan; have an annual Review of their Carers' Assessment and Support Plan; are supported to take regular breaks from their caring role; are supported to improve their own health and wellbeing. Carers Assessment and Support Service (East Lancashire). The Service Provider will support and enable the health and wellbeing of Carers by ensuring Carers are identified at an early stage; are supported to identify themselves as 'Carers'; have access to a range of personalised information, advice and services that they need to support them in their role; are offered a Carers' Assessment and Support Plan; have an annual Review of their Carers' Assessment and Support Plan; are supported to take regular breaks from their caring role; are supported to improve their own health and wellbeing. Carers Assessment and Support Service (Countywide). The Service Provider will support and enable the health and wellbeing of Carers by ensuring Carers are identified at an early stage; are supported to identify themselves as 'Carers'; have access to a range of personalised information, advice and services that they need to support them in their role; are offered a Carers' Assessment and Support Plan; have an annual Review of their Carers' Assessment and Support Plan; are supported to take regular breaks from their caring role; are supported to improve their own health and wellbeing. Lancashire Advocacy Hub (Countywide). The Lancashire Advocacy Hub Service will be commissioned as one countywide contract. The independent advocacy service – to be known as the "Lancashire Advocacy Hub" (or "the Hub") will support individuals to protect and safeguard their rights under the law; be involved in decisions about their health, care and support needs and how those needs are met; have their voices heard in their interactions with health, care and support services; resolve their issues with public services; advocate on their own behalf where appropriate. Lancashire Carers Advocacy (Countywide). The Lancashire Carers Advocacy Service will be commissioned as one countywide contract. The service will provide all statutory advocacy for carers under the Care Act, will seek to increase awareness of the rights of carers under the Act, and liaise closely with the Lancashire Advocacy Hub, the Carers Assessment and Support Service and other council-procured services (like the Wellbeing Service) to promote the availability of and engagement with the new Carers Advocacy service.
