Tender description

The provision of a service to ensure young carers are identified, registered and receive a whole family assessment under the statutory duty of the Council. In doing so, the service will contribute to improving health, education and social outcomes for children and young people who are identified as being a young carer, and reduce inappropriate caring roles undertaken by young carers. The service will comprise 4 components: Identification of young carers, voice of young carers, Whole family assessment and support for young carers, preparation for adulthood and transition. The service will comprise 4 components: — identification of young carers, — voice of young carers, — whole family assessment and support for young carers, — preparation for adulthood and transition. The estimated total value of the contract is stated in II.1.5) of this notice and is for the initial term inclusive of the approximate value for extensions, together with the their possible durations. For the avoidance of doubt, the estimated annual value of the contract will be approximately 273 279,00 GBP. The contract will be awarded for an initial term of 3 years, with the option to extend the contract up to 2 further years. Please note that the contract requirements will be subject to available financial resources, supplier performance and flexibility to meet changing demands. Organisations should be aware that due to the nature of the services provided, any Contract formed as a result of this procurement process shall be executed as a deed.
