Tender description

The contract is to appoint a Lead provider to deliver the families team element of the resilient families service. The families team will be allocated those families identified as requiring early intervention following assessment i.e. coordinated multi-agency support as required. These families are most likely to have been identified as benefiting from support via the pre-emptive referral route or been transferred from the families plus team. The families team will comprise of Intervention workers, who will work alongside other partners to ensure that: — the intervention plan is effectively delivered, — all professionals and services involved in delivering aspects of the plan are effectively coordinated and supported to deliver in a timely manner, — they provide a central point of contact for families and professionals and are proactive in addressing issues that may lead to either parties disengaging from the process.
