Tender description

Durham County Council recognise the need for a holistic support model that will help to create a pathway of provision for single males and females who are at risk of or experiencing homelessness. A person centred approach to those who would struggle to access accommodation without the help and assistance from support services. The Service will provide different levels of support in a range of accommodation provision, to those clients in need who have presented to Housing Solutions. This homeless support service will be delivered as one service with 4 distinct interlinking elements: • Assessment centre – including temporary accommodation • Supported Accommodation • Outreach with attached support • Independent living (support from Housing Solutions) Keywords: Homeless support
