Tender description

Provision of a new model of integrated anti-coagulation services across Croydon. The contract will be for 3 years with a reservation of rights to extend the contract for a further 6 months. The service will allow testing and piloting of a coordinated approach across secondary and primary care settings, through the Croydon GP Collaborative in partnership with Croydon Health Services NHS Trust. The contract is being awarded to Croydon Health Services NHS Trust in partnership with the Croydon GP Collaborative, which supports the CCG’s vision to integrate service provision across the patient journey to ensure seamless care. This aligns with the National policy drive to develop integrated care systems. This is intended to improve clinical outcomes and patient experience while ensuring that resources are used efficiently and that the system is financially sustainable. The pilot will further improve the model of care.
