Tender description

North of England Commissioning Support (NECS) are working for and on behalf of NHS Northumberland CCG who wish to commission a JMAPS service for Northumberland patients and are inviting expressions of interest in this tender opportunity. The overarching outcome of the service is to emphasise self-management to minimize the proportion of patients requiring intervention or onward referral. The service will reduce handoffs and improve patient outcomes. The service will provide a robust triage service and where treatment is required this will be delivered within the service or referred on to secondary care giving patient choice The service will have the following objectives: — provide clinical assessment, treatment and clinical leadership within a comprehensive community service, — provide robust triage through a single point of access, — enhance the management of patients within the community, and actively manage the demand for secondary care services ensuring patients have prompt access to appropriate treatments, — to provide or arrange, where necessary, relevant investigations e.g. MRI, X-ray, bloods and nerve conduction studies, — provide bio-psychosocial assessment of patients referred with chronic pain, — aim to de-medicalise chronic pain by implementing a model led by psychologists but incorporating APPs, physiotherapists and GPwSIs and other health workers with psychological training relating to pain management with the aim of decreasing the reliance on medication and hospital based interventions for the management of chronic pain, — to identify patients who are appropriate for surgical intervention and make onwards referral to their provider of choice, — to ensure patients are fit and prepared for the next stage of their preferred treatment, — to identify or exclude serious pathology and refer immediately/urgently as appropriate if identified, — to identify patients who require assessment by other services e.g. neurology, orthopaedics, rheumatology, podiatry, occupational therapy and arrange for patients to be referred appropriately, — to work closely with consultants, in the relevant sub-specialities, to ensure effective care for patients and develop agreed models of care for common musculoskeletal conditions and for pain management, — use SDM to deliver support and programmes to patients that fit with their personal goals of treatment, — enable patients to take ownership and to adopt strategies of living, which allow them to lead a fulfilling and independent life as possible with less reliance on the health care system, — work with GPs to increase their knowledge and confidence of diagnosis and management of MSK and pain conditions, — encourage collaborative working with other parties contracted to the CCG, — to ensure patient choice is delivered at the point of (onward) referral to secondary care; and — support the development of robust clinical governance and clinical audit processes
