Please include : 1)The name of your organisation 2) Contact details including email Description Provision of project support to 6 Sustainable Transformation Partnerships (STP's). The project support will develop the comprehension of, and action planning for, frailty pathways across an STP system. This should be delivered through personalised sessions facilitated by frailty experts to review each stage of the older persons care pathway to include: 1. Community services that are able to provide a crisis response to prevent admission 2. Direct ambulance conveyances to ED and potential for diversion 3. Ambulatory care provision 4. Admission avoidance strategies in ED, including frailty provision 5. Delays that occur in relation to pathway determination 6. Preparation for discharge and patient choice 7. Intermediate care provision and re-ablement 8. Care sector provision and trusted assessor. Maximum budget £25,000
