Tender description

To improve and prevent mental ill health among adults from African, Caribbean, Dual Heritage backgrounds (male and female), through the provision of a holistic culturally sensitive community support, outreach and carers service. The service will work with a number of individuals and therefore a range of presentations, which will mean working successfully with people who may have psychosis, a forensic or criminal history. The service will aim to keep people well in the community and reduce the risk of hospital admissions or escalation into specialist services where appropriate.
