Tender description

The purpose of this care at home service is to support people and where appropriate, support their carers to have a good quality of life in their own homes. The service covers both long term care needs and short term interventions which may include personal care and social/emotional support. In line with reablement/enablement practices, the service aims to support people to sustain, recover and/or manage their health, skills and abilities to lead independent and fulfilling lives. The service also provides support to those approaching the end of their lives in their own homes, where this is their choice.
