Assess Young Carer Needs and Outcomes — ensure statutory Young Carer assessments are carried out, using the Early Help process to identify and co-ordinate support across agencies; to include wider support for the family such as Adult Social Care. Work in partnership with children and families social care – by, where appropriate, supporting the completion of assessments being carried out under section 17 and 47 of the Children Act 1987. Use tools such as the manual for measures Maca and Panoc to assess and regularly review young carers’ needs for support, managing plans for independence and closure from the service. Multi agency planning – develop a caseload system and act as lead professional where appropriate, working in partnership with all agencies, attend planning meetings and develop plans. Provision of a young carers support service for children and young people within South Tyneside. The Service to be provided will be within South Tyneside.
