Tender description

Derbyshire County Council is seeking a provider for the design and delivery of a self-management programme or people living with Long Term Conditions (“Programme”). The intervention will be designed to empower people hat require support to better manage their Long Term Condition (LTC) through a self-management model. The Programme must be available to people within the geographical area of the Derbyshire (“County”), with the exception of Derby City. In line with the national “Talking Health” approach the programme will target people living with LTCs; with face-to-face course sessions being delivered by trained volunteer tutor peers (themselves living with LTCs). The initial contract period is for 2 years commencing on 1.12.2018 to 30.11.2020, with the option to extend for a further 2 x 12 month periods.
